Highway to Health

Highway to Health (H2H) is a mobile outreach van that is case management driven. The mobile outreach van travels throughout the county to provide screenings and linkage to services such as behavioral, medical, and social. This free program was set up by the Health Department to help clients get assistance with needs that they may have had trouble navigating in the past. Clients will be able to connect to health department services without traveling to the health department. The coordinators also are able to link clients to other needed services outside of the health department. Calvert County Health Department offers many programs to promote and improve health and wellness for community members. If you would like more information about our programs, please fill out the Community Health Programs Inquiry Form and a staff member will contact you within two business days.
